Synergized Communities Thrive, Inc. is a 501(c)(3) nonprofit dedicated to health equity in Lake County, Illinois.
Mission
To provide free, evidence-based whole health education and wellness programming to underserved communities in Illinois, empowering individuals of all ages and life stages to achieve and sustain optimal health and wellbeing.
Vision
A Lake County where every community, regardless of zip code, has access to the wellness resources, education, and coaching needed to live thriving and connected lives.
Who we serve
Community-first, at every life stage.
Youth (14–18)
Health and wellness literacy for teens in underserved Lake County communities.
Active Adults (18–55)
Primarily working women navigating stress, lifestyle change, and long-term health.
Ageless and Awesome (55+)
Muscular and cardiovascular endurance, weight management, metabolic health, deep health coaching, and social connection for retired and semi-retired adults.
Nonprofits & Schools
Park districts, senior centers, and schools bringing free programs to their sites.
Grant Funders
Investors in health equity looking for transparent, measurable community impact.
Donors
Individuals who believe zip code should not determine access to whole health.
All programs are free to qualifying participants in Lake County. Paying organizations are served through Synergized Coaching, Inc. (SC, Inc.), SCT's for-profit sister organization.
